Cancel Your Appointment
If you are unable to attend your booked appointment with the nurse / doctor / phlebotomist, please let us know as soon as possible so we can make this slot available for another patient. We request that you give us a minimum of 24 hours notice for any cancellation except in the case of an emergency.
Request Prescription
Standard Prescriptions after medical review are for a period of six months. Staff will contact you if updated blood tests or a medical review is required.
Please allow 48 hours for prescription to be sent to your pharmacy
A charge will apply for patients without medical cards
Request Routine Test Results
Please contact the clinic two weeks after completing tests to ensure no action is needed. A member of staff will get back to you within 48 hours by phone or text to update you about your results.
Request Sick Cert / Social Welfare Cert / Letter
This request may be used for ongoing social welfare certs or for a new illness. In the case of a new illness, we will need to contact you to get more details and a fee will apply if a private cert for work is required.
If a fee applies, a link will be sent to you by text / email for payment prior to the cert being issued.
Social welfare certificates should only be requested if you intend to submit an illness benefit application to the dept of social welfare in order to claim illness benefit. The form can be completed HERE.
